Referral and Assessment Process +

HH ltd accepts referrals for admissions ages 18-65

Below are the steps outlined for each referral received.

Needs assessment

Series of observations will be made on an agreed date and time to assess the needs of the service user. Family and friends are welcome to help provide as much information as possible

Residential Visit

A date will be set in the diary for the individual requiring admission to see the premises alongside any healthcare professional, family, and friends, ensuring all parties are happy.

Costing

Harmony homes limited will send the costs of providing care to the placement authority. The placement authority will have to agree to the costs before contracts are signed.

Induction

An agreement plan will be made between the service user and other parties involved. An induction will be carried out with the individual prior to using the facilities.

What to expect +

After the final meeting the manager will assign a key worker who will be responsible for the care user. An action plan will then be formulated including addressing any health concerns.

Throughout the care users time at Harmony Homes- the key worker will liaise with the manager at the following intervals: 6 weeks, 3 months, 6 months and yearly.

Eating and Living

We believe an essential part of supported living is to help the individual service user to create their own menus and carry out their weekly shop accordingly.

Individuals are encouraged to carry out an active lifestyle, balancing physical health alongside mental health. We encourage individuals to go for walks or join the local gym.

Sutton has a thriving diverse cultural background, and every individual is given the opportunity to practise their faith. Harmony homes supported living is located opposite St Johns Church, serving its community
